Te Ara Pae Ora is a health and hauora app to support whānau to connect to local health, social and community services that can support them in the areas that they need. The app gives users control about the information they want to share with other support people or service providers.

The app has provision for users to create a hauora wellness plan with their care team. The plan helps your healthcare providers to understand your hauora (health and wellbeing) and support you to be as well as you can be. 

Currently the app is only available for people living in the Kāpiti, Porirua, Wellington and Wairarapa regions.
