The Frailty care guides | Ngā aratohu maimoa hauwarea are a suite of 26 nursing decision support tools covering the spectrum of frailty from deterioration to communication and advance treatment planning.
They are designed to complement best practice and promote early intervention and communication with the person’s healthcare team.
