The Ministry of Health's Long-Term Conditions Forum for DHBs and Primary Care will be held in Wellington on 27–28 February 2020. The purpose will be to showcase examples of innovative programmes that are improving outcomes for Māori and Pacific people, integrating care across services, consumer-centric care and improving mental wellbeing from across the country.
The Ministry of Health in partnership with Health Navigator NZ and Health Literacy NZ are organising a Long-Term Conditions forum to be held 27-28 February 2020 at Te Papa, Wellington.
The title of this year’s forum is 'Transforming Care.'
We want to share what is working locally and regionally, and discuss whether and how these experiences can be applied to other regions. Some of the work shared will focus on service improvement within existing systems, and other speakers will showcase innovation which has the potential to be applied more widely.
Themes for this forum include:
- Achieving equity
- Improving Māori health outcomes
- Improving Pacific health outcomes
- Integration across community, primary and secondary care
- Integration with social and other services (such as housing support)
- Consumer leadership and design
- Mental wellbeing
There will be a range of workshop presentations and posters highlighting the best projects and initiatives from around the country demonstrating integrated and coordinated services that lead to positive outcomes for people with long-term health conditions. We also want to highlight national priorities of achieving equity, mental wellbeing, and taking a person and whānau-centred approach to care.
Audience
The intended audience are planning teams and long-term conditions clinical leads from DHBs and Primary Care, Maori, Pacific providers and community or primary teams.
Places are limited, so apologies, registrations are invite only. DHBs and PHOs will receive an invitation early December 2019.
