Health Navigator NZ is a national charitable trust meaning we are a non-profit organisation. Our purpose is to support families, carers, health providers and health organisations to improve the prevention and management of long-term health conditions such as diabetes, arthritis, depression and much more.

We are doing this through a range of approaches including:

  • Through this website - providing reliable health information at a range of literacy levels
  • Workforce development - training and resources for general practice, hospital and community teams in chronic care and self management support
  • Clinical Networks - see below
  • Advocacy and policy to support transformational change
  • Resources for healthcare providers - care plans, self management toolkit and much more
  • Facilitating collaboration and knowledge sharing
  • Production and distribution of the Long-Term Conditions Bulletin
  • Organisation of local, regional and national events such as the Australasian Long-Term Conditions Conference
  • Brokering relationships across organisations to reduce fragmentation and duplication

 

 

 

We have also established the Long-Term Conditions Network as a forum for supporting knowledge exchange, networking, virtual communities and regional events for both clinicians and members of the public interested in working together to improve care and outcomes. Like many charities, Health Navigator NZ and the Long-Term Conditions Network must continually fundraise to ensure its services meet the needs of those it supports.
