Care Plans & Action Plans

Action plans provide you with written information reminding you what to do when things go wrong or symptoms occur. These can be very useful for a range of conditions such as asthma, diabetes, heart failure, COPD (Chronic Obstructive Pulmonary or Lung Disease), gout, bothersome eczema or significant allergies.

  • Knowing what to watch out for (your early warning signs) and what you can do at an earlier stage can keep you out of hospital
  • Ask your doctor or nurse for an action plan and keep it somewhere safe so you and your family know what to do
