| National Shared Care Plan Programme
The National Shared Care Plan Programme is a programme in the Auckland region aiming to improve integration of care across primary, hospital and community settings. The programme provides summary health information and an electronic care plan 24/7 that is accessible via the internet, hospital systems or general practice systems for people with long-term health conditions. The care plan is developed with the patient and their family/whanau (where desired), they can view it via a secure portal and includes mutually agreed problems, goals, actions, timeframes and accountabilities, ie who will do what and by when. Nutrition FactsheetsAuckland District Health Board has developed some excellent easy to read factsheets on a range of topics as part of the Refugee Health Collaborative they have been doing.These can be downloaded from the Tools & Resourcessection and languages include Arabic, Amharic, Burmese, Dari English and Somali.

Mental Health - MoodGYM - Online CBT for Prevention of Depression MoodGYM is a FREE online Cognitive Behaviour Therapy (CBT) programme designed for the prevention of depression. The innovative and interactive web programme provides users with online exercises, an interactive game, anxiety and depression assessments, downloadable relaxation audio, a workbook and feedback assessment that can be completed over 5 modules. Developed by the Australian National University, MoodGYM teaches the principles of cognitive behaviour therapy – a proven treatment for depression. "It also demonstrates the relationship between thoughts and emotions, and works through dealing with stress and relationship break-ups, as well as teaching relaxation and meditation techniques."

Book - Managing Chronic Illness by Veronica Latham This is an excellent book for anyone living with a long term condition or illness such as asthma, diabetes, chronic pain, arthritis, depression and heart problems. "Sympathetic, effective and informative, the book provides practical advice on steps you can take to reclaim your health and take control of your life again." The author is able to bring a unique perspective and understanding, both as a patient living with a long-term condition and as a health professional. Well worth reading.
