Background

There is growing interest in the area of long-term conditions and its contribution to acute demand, escalating costsConference-Flyer and overloaded hospitals, yet there are few opportunities for primary, secondary and tertiary care clinicians, funders, programme managers, service providers, community organisations, researchers and policy analysts to meet, share ideas, knowledge or learnings.

Health Navigator Charitable Trust in collaboration with the Auckland region District Health Boards (Auckland, Counties Manukau, and Te Whariki Teitei (Rural Centre of Excellence being established by Waitemata DHB) and the Heart Foundation are pleased to be hosting this unique conference as an opportunity to facilitate these discussions and knowledge sharing.
